To redeem your free books, visit www.scholastic.com/kumon and download the age-appropriate activity sheets, and completely fill out and mail to: Kumon Book Offer, 557 Broadway, P.O. Box 711, New York, NY 10013. Include your name, daytime phone number, email address, complete mailing address and the ages of your children. Sending in one (1) completed activity sheet(s) will earn you two (2) free books (a "Set ") and sending in two (2) completed activity sheets will earn you five (5) books. Maximum books allowed is five (5) per household. You can only send in activity sheets if you are the parent and/or legal guardian of a child who falls within the activity sheet age groups (ages 5-6, 7-9 or 10-12). Two activity sheets maximum per age group per participant - one reading, one math.
